New patients are asked to arrive 30 minutes early so that the provider can review initial paperwork. Late arrivals cannot be accommodated, sorry.
Accepted methods of payment are cash, credit, debit, FSA/HSA or VA referral. Health insurance is not accepted. Payment is due at the time of service. Office hours are Monday -Thursday 8:30am-3:30pm
